Your First Visit
To ensure a smooth first visit, please fill out the forms online beforehand. If you’re unable to do so, we kindly ask that you arrive 15 minutes early to complete the necessary paperwork in the office. Bring your ID, insurance card, and medical history. Dr. Storer will conduct a comprehensive exam, including X-rays if needed, and discuss your dental goals. We'll create a personalized treatment plan and answer all your questions as well.
